How Long Does Mounjaro Stay in Your System?
After the last injection, how long does Mounjaro keep working? The answer is longer than most people expect, because tirzepatide is designed to be given just once a week. This guide explains what “staying in your system” means, the half-life of tirzepatide, how long it takes to clear, why appetite and weight do not change overnight when you stop, and what this means for pregnancy planning, surgery and other medicines.
Quick Answer

Mounjaro stays in your system for roughly a month after your last injection. Tirzepatide, the medicine in Mounjaro, has a half-life of about 5 days, which means half of it is cleared roughly every 5 days, and after around five half-lives (about 25 days) very little remains. That is why one weekly dose works for a full week and why effects fade gradually after you stop rather than suddenly. The UK product information advises stopping at least 1 month before a planned pregnancy because of this long half-life. For anything where timing matters, such as surgery or pregnancy, speak to your prescriber rather than relying on an estimate.

What “Staying in Your System” Means

When a medicine is in your system, measurable amounts are still circulating in your blood. A medicine can remain in your blood after its noticeable effects fade, and noticeable effects can persist while levels fall. Two ideas help here. The half-life is the time it takes for the level in your blood to fall by half. The clearance time is how long it takes to fall to a level that no longer has any meaningful effect, which is usually estimated at about five half-lives.

The Half-Life of Mounjaro

The UK Summary of Product Characteristics for Mounjaro refers to the half-life of tirzepatide as approximately 5 days. This long half-life is the reason the medicine can be injected once a week. After one dose, the level in your blood rises, peaks over the following day or two, and then falls slowly across the week. By the time the next injection arrives, there is still some medicine left, and with weekly dosing the level builds to a steady state over several weeks.

25 days (5 half-lives)About 3%Mostly cleared

These percentages are a simple calculation from a 5-day half-life, not a measured figure for every person. Individual differences such as body size, kidney function and dose history mean the real pattern varies. They do, however, show why “about a month” is a sensible rule of thumb for the medicine being largely gone.

Why You Do Not Feel the Effects Stop Immediately

Many people expect appetite to bounce back within days of a final dose. In reality, because the medicine declines gradually, the change is usually gradual as well. You may notice hunger returning in the second or third week after the last injection, food noise creeping back, or portion sizes growing. Some people feel little difference for the first week. If you have stopped or are thinking of stopping, our guide to what happens when you stop Mounjaro, Wegovy or Foundayo covers appetite, weight regain and the options for maintenance.

How Long Do Side Effects Last After Stopping?

Side effects follow the same gradual decline. Digestive symptoms such as nausea, constipation or diarrhoea usually settle as levels fall, although this can take a few weeks. If you stop for a few weeks and restart, digestive effects can return, and the Wegovy product information says a lower dose should be considered when restarting after several missed doses, so prescribers often advise a lower restart dose after a longer break. Our guide to missing a Mounjaro dose explains the practical rules for a single late or skipped dose.

Pregnancy Planning

This is one of the most important reasons to know how long Mounjaro lasts. The UK product information states that tirzepatide should not be used during pregnancy and should be discontinued at least 1 month before a planned pregnancy because of its long half-life. If you are trying to conceive, speak to your prescriber well in advance so that you can plan a safe stop. If you become pregnant while taking Mounjaro, stop the injections and contact your prescriber or GP promptly.

The product information also notes that tirzepatide could be considered for use during breastfeeding, but the decision should be made with your prescriber. Mounjaro can also affect the contraceptive pill. The product information advises switching to a non-oral method or adding a barrier method for 4 weeks after starting and after each dose increase. Our guide to Mounjaro and the contraceptive pill gives the details.

Surgery, Anaesthesia and Procedures

GLP-1 medicines slow the emptying of the stomach, which matters for procedures needing sedation or anaesthesia because of the risk of food or fluid being aspirated into the lungs. The MHRA issued a Drug Safety alert in January 2025 about this, and UK perioperative guidance published in 2025 reflects a more individualised approach than the earlier blanket advice to stop for a fixed period. Because the medicine lasts so long, simply stopping a week or two beforehand does not remove it from your system, which is one reason clinicians use risk assessment and specific precautions instead. Our guide to having surgery on Mounjaro, Wegovy or Foundayo explains this in full, including what to tell your team.

Tell your team

If you are having surgery, an endoscopy or any procedure with sedation or anaesthesia, tell the clinical team that you take Mounjaro and when you last injected. Do not stop or change your doses without their advice.

Other Medicines

Because tirzepatide slows gastric emptying, it can affect how some oral medicines are absorbed. The product information describes a reduction in the peak level of paracetamol of around 50% in a study, with a delay in absorption, and advises monitoring for medicines with a narrow therapeutic index, such as warfarin and digoxin. The effect is most pronounced after the first dose and after dose increases and lessens over time. If you take any critical tablets, tell your pharmacist about your Mounjaro, including when you last injected.

Alcohol, Blood Tests and Drug Tests

There is no special wash-out time for alcohol, but alcohol can worsen nausea, reflux, dehydration and low blood sugar, particularly when you are eating little. Our guide to alcohol on Mounjaro explains this. Routine blood tests do not usually look for tirzepatide, and standard workplace drug tests do not screen for it. If a particular test matters to you, ask the organisation or your clinician what it covers.

Semaglutide’s half-life is about a week, so it takes slightly longer to clear. The Wegovy product information advises stopping at least 2 months before a planned pregnancy, compared with 1 month for Mounjaro. Daily tablets such as the Wegovy pill and Foundayo have different timings, which are covered in their own guides.

Common Misunderstandings
  • “It is out of my system after a week.” Only about half is cleared in around 5 days, so a week-old injection still has a significant effect.
  • “Once it has gone, my appetite will be exactly as before.” Many people notice hunger and food thoughts returning, but this is individual and happens gradually.
  • “Skipping a dose clears it faster.” It does not; stopping and restarting can bring back side effects.
  • “I can estimate my own clearance date for surgery.” Surgical and pregnancy timing should always be decided with a clinician.
Practical Checklist
  • Note the date of your last injection and share it with any clinician treating you.
  • If you plan pregnancy, speak to your prescriber early and plan at least a month ahead.
  • If you stop, plan for the return of appetite with protein, structure and support.
  • If you restart after more than a short gap, ask about a lower restart dose.
  • Report any unexpected symptoms rather than assuming they are from a medicine you have already stopped.

The bottom line

Mounjaro stays in your system for about a month after the last injection, because tirzepatide’s half-life is roughly 5 days. That is why it works for a week from one dose, why effects fade gradually, and why pregnancy planning needs at least a month’s notice. For surgery, other medicines or anything time-sensitive, work out the timing with your prescriber rather than estimating.
