Mounjaro Face: Facial Changes During Weight Loss
Many people notice their face looks thinner, more hollow or more lined as the weight comes off, and the change is often called “Mounjaro face”. For some it is welcome. For others it is a surprise. This guide explains why faces change during weight loss, what is and is not specific to GLP-1 medicines, what can help, and when a change is worth a medical check.
Quick Answer

Mounjaro face is a popular term for facial thinning, hollowing, sagging or deeper lines that some people notice during weight loss on Mounjaro. It is not a medical diagnosis and it is not unique to one medicine, because the face loses fat just as the rest of the body does. Changes are more likely to show with larger or faster weight loss, with age, or when skin is less elastic. Steadier weight loss, enough protein, strength training, sun protection and good skin care can help, and sudden swelling or one-sided changes should be checked by a clinician.

What People Mean by Mounjaro Face

The phrase usually covers a mix of the following.

  • Cheeks that look flatter or hollower
  • Temples and eye sockets that look more sunken
  • A sharper jawline, or looser skin under the chin
  • Deeper smile lines from nose to mouth
  • Skin that looks thinner or less plump

Some people see these as signs of getting leaner. Others feel they look older or unwell. Both reactions are understandable.

Why the Face Changes When You Lose Weight

The face contains small pockets of fat, called fat pads, that sit under the skin and over the bones. They give the face its fullness and help to support the skin. When you lose weight you lose fat from all over the body, including these pads, and the skin has to adapt to a smaller volume. Younger, more elastic skin tends to retract. Skin that is less elastic, because of age, sun exposure, smoking or genetics, may not, and that can look like sagging or deeper lines.

So facial change mostly reflects how much and how quickly weight is lost, not one particular medicine. The same changes are reported after bariatric surgery and very low calorie diets. In the main Mounjaro trial in adults with obesity, average weight loss at 72 weeks was about 15% on the lowest maintenance dose and about 21% on the highest (SURMOUNT-1). Losing that proportion of body weight is likely to change the face as well as the rest of the body. Our guide to Mounjaro before and after results shows what to expect overall.

What Is and Is Not Known
QuestionWhat we knowWhat we do not know
Does Mounjaro itself affect the face?Weight loss changes facial fatWhether the medicine has any effect on the face beyond weight loss
Is it more common with GLP-1 medicines?Large average weight loss makes changes more visibleNo trial has measured facial change as an outcome
Who notices it most?Older adults, people with less elastic skin and those losing a lot of weight quicklyThe exact amount of loss that causes visible change
Does it reverse?Weight regain restores facial fat for some, and skin can improve over timeHow much skin recovers in different people

No one can predict exactly how your face will change. The sensible approach is to plan for the possibility and look after your skin and general health, rather than treating it as a complication that needs fixing.

What You Can and Cannot Influence
Things you can influence
  • How quickly you lose weight, with your prescriber
  • Protein and overall nutrition
  • Strength training
  • Sun protection and skin care
  • Smoking, vaping and alcohol
  • Sleep and hydration
Things you cannot control
  • Age and natural skin elasticity
  • Genetics and family patterns
  • Your starting face shape
  • Past sun damage
  • How your body distributes fat
What Can Help

Aim for steady weight loss

If you are losing weight very quickly and are worried about how you look or feel, speak to your prescriber. Sometimes staying longer on a dose before increasing is appropriate. This is a normal adjustment, not a failure. Do not reduce or skip doses on your own.

Eat enough protein

Skin, hair and muscle all rely on protein. A 2025 US joint advisory on nutrition during GLP-1 treatment proposes 1.2 to 1.6 grams of protein per kilogram of body weight per day during active weight loss, and warns that very low calorie intakes make it hard to get enough vitamins and minerals. This is US guidance, so check your own target with a clinician or dietitian. Our protein guide and guide to what to eat on Mounjaro or Wegovy have practical ideas.

Do strength training

Resistance exercise protects muscle and supports posture and body shape. It will not rebuild facial fat, but feeling strong often changes how people see themselves. See our guide to protecting muscle on Mounjaro.

Look after your skin

  • Use a broad-spectrum sunscreen (SPF 30 or higher) every day. Sun damage is the biggest controllable factor in skin ageing.
  • Moisturise regularly and keep fluids up.
  • Ask a pharmacist about gentle, evidence-based products. Retinoids can improve skin texture but can irritate, and they are not suitable in pregnancy.
  • Avoid smoking and vaping, which speed up skin ageing.
  • Keep alcohol low, because it dehydrates the skin and disrupts sleep.

Skin elsewhere changes too. Our guides cover loose skin after weight loss and hair loss on Mounjaro.

Cosmetic Options

Some people choose cosmetic treatments to restore volume or improve skin quality. They are optional, and none is needed for your health.

OptionWhat it aims to doPoints to consider
Dermal fillersRestore volume in cheeks or templesRegulated practitioners only; best once weight has stabilised
Skin-quality treatments (peels, microneedling, lasers)Improve texture and firmnessResults vary; ask about suitability for your skin
Skin-tightening devicesStimulate collagenEvidence varies; be wary of exaggerated claims
SurgeryRemove excess skin or reposition tissueA specialist decision, usually once weight is stable

Choose regulated practitioners

Cosmetic injectables should only be given by suitably qualified, regulated healthcare professionals in a registered setting. Complications from poorly performed filler treatment can be serious. Ask about training, insurance and what happens if something goes wrong.

Frequently Asked Questions
What is Mounjaro face?
A popular term for facial thinning, hollowing or looser skin that some people notice during significant weight loss. It is not a medical diagnosis.
Does Mounjaro make you look older?
Weight loss can make some faces look older, because facial fat pads shrink and skin may not fully retract. It depends on how much and how fast you lose weight, and on your skin.
Is Mounjaro face permanent?
Not necessarily. Some people’s skin improves over months, and regaining weight restores facial fat. Cosmetic options exist for those who want them.
Can I prevent Mounjaro face?
Not completely, but steady weight loss, enough protein, strength training, sun protection and skin care can make it less noticeable.
Who is most likely to notice it?
Older adults, people with less elastic skin, and those who lose a large amount of weight quickly.
Does it happen on Wegovy too?
Yes. It is reported with weight loss by any method, including Wegovy, surgery and diets. “Ozempic face” is used for the same thing.
Should I slow down my weight loss?
If you are worried, speak to your prescriber. Sometimes staying longer on a dose is appropriate. Do not change doses yourself.
Do fillers help?
They can restore volume for some people, but only regulated practitioners should give them, and ideally once your weight has stabilised.
Does collagen help?
Evidence for collagen supplements is limited. Start with protein, sun protection and good nutrition, and ask your pharmacist before buying supplements.
Will drinking more water help?
Staying hydrated supports your general health and skin, but water alone will not restore lost facial volume.
When should I worry about a change in my face?
Call 999 for sudden swelling, drooping or difficulty breathing. See a clinician for new lumps or swelling or pain that does not settle.
Will my face look better once my weight settles?
Many people find their appearance settles once weight stabilises and the skin has time to adapt, although results vary.
