Mounjaro Fatigue: Why You Feel Tired and What Helps
Feeling tired on Mounjaro or Wegovy is far more common than most people expect. This guide explains why tiredness happens, what the UK product information says about how common it is, how to tell ordinary treatment fatigue from something that needs checking, practical steps for food, fluids, sleep and activity, and the warning signs that mean you should speak to a clinician.
Quick Answer

Mounjaro fatigue is common: the UK product information lists fatigue as a very common side effect of Mounjaro, and the same applies to Wegovy. In most people it comes from a combination of eating much less than usual, mild dehydration, disrupted sleep, the body adjusting to a new dose, and sometimes low iron, vitamin B12 or blood sugar. It often improves within a few weeks or once a dose settles. What helps most is enough protein and calories, steady fluids, regular sleep, gentle movement and strength work, and checking any underlying causes with your prescriber. Speak to a clinician if tiredness is severe, lasts weeks, or comes with dizziness, fainting, palpitations or low mood.

How Common Is Fatigue on Mounjaro and Wegovy?

The UK Summaries of Product Characteristics list fatigue as a very common side effect of both Mounjaro (tirzepatide) and Wegovy (semaglutide). Very common means it affects more than one in ten people. In the product information, fatigue is grouped with related terms such as tiredness and lack of energy, which is how people tend to describe it. Feeling sick or not eating well also drains energy, so digestive symptoms and tiredness often go together.

The product information does not describe fatigue as a sign of a dangerous problem. It is typically a side effect that comes and goes. But because it can have several contributing causes, it is worth working out which ones apply to you rather than simply waiting.

Why GLP-1 Medicines Can Make You Tired

Eating much less

These medicines reduce appetite and slow stomach emptying, so many people eat far less than they used to. If your calorie intake falls sharply, your body has less fuel and tiredness is a natural result. A 2025 US joint advisory on nutrition during GLP-1 treatment notes that very low intakes (below about 1,200 kcal a day for women and 1,800 kcal a day for men) make it difficult to get enough vitamins and minerals. This is US guidance, but the principle holds: undereating is one of the most common and most fixable causes of tiredness on treatment.

Not enough protein

Low protein intake can leave you feeling weak and drained, and it contributes to lean mass loss during weight loss. The same advisory proposes 1.2 to 1.6 grams of protein per kilogram of body weight per day during active weight loss, with a simpler practical target of about 80 to 120 grams a day. Our protein guide has food lists and meal ideas.

Dehydration

You may feel less thirsty on these medicines, and digestive side effects such as vomiting and diarrhoea lose fluid. The product information notes that digestive reactions may lead to dehydration, which in rare cases can affect kidney function. Even mild dehydration causes headache, dizziness and tiredness. Our guide to diarrhoea, bloating and reflux covers what helps.

Dose changes

Tiredness often peaks in the days after starting treatment or increasing the dose, then settles as your body adjusts. If you notice a pattern linked to injection day, that is a useful clue to share with your prescriber.

Poor sleep

Nausea, reflux, a dry mouth or an early-morning bathroom trip can interfere with sleep, and disturbed sleep is an often overlooked reason for daytime tiredness. Our guide to sleep issues on Mounjaro goes into more detail. Weight loss can also reduce snoring and sleep apnoea over time, but if you wake unrefreshed or a partner notices pauses in your breathing, mention it to your prescriber.

Low blood pressure

Hypotension-related events and dizziness are listed as common with Mounjaro. As weight falls, blood pressure often drops, which is usually helpful, but if you also take blood pressure medication it may become too low, leaving you light-headed and weary.

Low blood sugar

On their own, these medicines rarely cause low blood sugar. In the Mounjaro product information, hypoglycaemia is very common when it is used with a sulphonylurea or insulin. If you have diabetes and feel shaky, sweaty or confused as well as tired, speak to your prescriber urgently, because your other diabetes medicines may need adjusting.

Nutrient gaps and other medical causes

Eating less can lower iron, vitamin B12, vitamin D and other nutrients. Separate conditions can also cause tiredness at the same time, including anaemia, an underactive thyroid, depression and infections. This is why persistent fatigue deserves a conversation rather than assumptions.

What Helps: A Practical Plan

Fuel properly

  • Eat at regular intervals even if you are not very hungry. Three small meals and a snack are often easier than waiting for hunger.
  • Put protein first at every meal: eggs, yoghurt, fish, chicken, tofu, beans or a shake on low-appetite days.
  • Include slow-release carbohydrates such as porridge, wholegrain bread, potatoes or pulses for steady energy.
  • Keep a few easy foods ready for low-energy days so you do not skip meals.

For more ideas, see our nutrition guide for Mounjaro and Wegovy.

Drink steadily

  • Sip through the day rather than gulping when you remember.
  • Carry a bottle and aim for pale yellow urine.
  • If you have diarrhoea or vomiting, consider an oral rehydration solution and seek advice if you cannot keep fluids down.
  • Limit alcohol, which dehydrates and disrupts sleep.

Protect sleep

  • Keep a regular bedtime and wake time.
  • Avoid large meals late in the evening, which can aggravate reflux and disturb sleep.
  • Raise the head of the bed slightly if reflux wakes you.
  • Avoid caffeine in the afternoon and evening.

Move, including strength work

  • A short walk can lift energy better than resting when you feel flat.
  • Add resistance exercise two or three times a week to protect muscle, using bodyweight, bands or weights.
  • Build up gradually. If a workout leaves you drained for a day, scale back and increase more slowly.

Talk about timing and dose

If tiredness is clearly linked to a dose step, your prescriber may suggest staying on the current dose for longer before moving up, or in some cases a smaller step. This is a normal adjustment and not a failure. Never skip or change doses on your own. Our Mounjaro dose guide explains how the steps work.

How Long Does Treatment Fatigue Last?

There is no fixed number. Many people find tiredness eases within a few weeks of starting or after a dose settles, as intake stabilises and the body adjusts. A simple way to approach it:

  1. 1Days 1 to 3 after a doseExpect some flatness. Keep up food and fluids, and note the pattern.
  2. 2Weeks 1 to 4Review protein, fluids and sleep using the plan above.
  3. 3After 4 weeks on a stable doseIf tiredness is still significant or getting worse, contact your pharmacist or prescriber.
  4. 4Any timeSevere tiredness or warning symptoms need prompt advice.
When to Get Advice

Speak to a clinician

Seek advice if your tiredness is severe, lasts several weeks, or comes with dizziness or fainting, a fast or irregular heartbeat, chest pain, shortness of breath, confusion, persistent low mood, or an inability to keep fluids down. Severe tiredness with intense abdominal pain, yellowing of the skin or eyes, or repeated vomiting needs urgent help.

SituationWhat to do
Mild tiredness for a few days after a doseFuel, fluids and sleep; see if it settles
Tiredness lasting more than 2 to 4 weeksContact your pharmacist or prescriber
Tired with dizziness on standingReview fluids and blood pressure medicines
Tired with shakiness, sweating or confusion (on diabetes medicines)Urgent prescriber advice
Tired with palpitations, chest pain or breathlessnessSeek urgent medical help
Tired with persistently low mood or loss of interestSpeak to your GP or prescriber
Fatigue, Mood and Mental Wellbeing

Tiredness and low mood are closely linked. Weight loss can bring a mix of emotions, and changes in eating habits can affect how people feel. If fatigue comes with persistent low mood, loss of interest, anxiety or changes in sleep, tell your prescriber or GP. They can look at whether it is related to treatment, to other health factors, or both.

Questions to Ask Your Pharmacist or Prescriber
  • Could my dose or titration speed be contributing?
  • Should I have blood tests for iron, B12, vitamin D or thyroid function?
  • Do any of my other medicines need reviewing?
  • Is my calorie and protein intake adequate?
  • How long should I wait before I worry?

For the wider picture of what to expect, see our guide to Mounjaro side effects.

The bottom line

Fatigue is a very common side effect of Mounjaro and Wegovy, and it usually has identifiable and fixable causes: too little food or protein, not enough fluid, poor sleep, or a dose step that has not yet settled. Fuel regularly, drink steadily, protect your sleep and keep moving. If tiredness is severe, lasts more than a few weeks or comes with warning symptoms, speak to a clinician so that nothing else is missed.

Frequently Asked Questions
Does Mounjaro make you tired?
Yes, fatigue is listed as a very common side effect of Mounjaro. It is often linked to eating less, dehydration, sleep disruption and dose changes, and it frequently improves.
Does Wegovy make you tired?
Yes, fatigue is also listed as very common with Wegovy. The causes and the practical steps are the same.
How long does Mounjaro tiredness last?
Often a few days after an injection or dose increase and a few weeks overall. If it continues beyond about a month on a stable dose, ask for a review.
Why am I so tired after my Mounjaro injection?
Many people feel more tired in the days after injecting, particularly after a dose increase. Keeping up food and fluids and noting the pattern helps your prescriber advise you.
Can low calories cause fatigue on Mounjaro?
Yes. Eating very little is a common cause. Very low intakes also make it harder to get enough vitamins and minerals.
Can dehydration cause tiredness on Mounjaro?
Yes. Reduced thirst plus digestive side effects can lead to dehydration, which causes headache, dizziness and fatigue.
Should I take vitamins for tiredness?
A multivitamin may be considered if you are eating very little, but tiredness can have other causes. Ask your pharmacist or GP before starting supplements, and consider blood tests if it persists.
Can Mounjaro cause low blood sugar and tiredness?
On its own it rarely does, but the risk rises with insulin or a sulphonylurea. If you take these, seek urgent advice for shakiness, sweating or confusion.
Is exercise good for tiredness on Mounjaro?
Gentle regular movement usually helps energy. Build up gradually and include strength work, but rest if you feel faint or unwell.
Can I lower my dose if I am too tired?
Speak to your prescriber. They may suggest staying on the current dose for longer or adjusting your plan. Do not change doses yourself.
Could my tiredness be something else?
Possibly. Anaemia, thyroid problems, poor sleep, low mood and infections can all cause fatigue. Persistent tiredness deserves a medical check.
When should I worry about fatigue?
If it is severe, lasts weeks, or comes with dizziness, palpitations, chest pain, confusion or an inability to keep fluids down, seek advice promptly.
