Mounjaro Pancreatitis and Gallbladder Problems: Warning Signs
Mounjaro can be associated with acute pancreatitis and gallbladder problems, although most people will not develop either. Seek urgent medical help if you develop severe, persistent stomach pain that may spread to your back, with or without nausea or vomiting. Pain under the right ribs that lasts more than 30 minutes, jaundice, fever or repeated vomiting can point to a gallbladder problem and also needs prompt assessment. If pancreatitis is suspected, the official product information says treatment should be stopped. If pancreatitis is confirmed, it should not be restarted.
The main warning sign is pain that is clearly more severe and persistent than ordinary indigestion or short-lived stomach discomfort. NHS guidance describes acute pancreatitis as a serious condition that needs urgent treatment in hospital.
- sudden or severe pain in the upper abdomen that does not go away
- pain that spreads to the sides or back
- nausea or vomiting alongside the pain
- a high temperature, a fast heartbeat, persistent bloating or difficulty breathing
When to get help
Contact NHS 111 urgently if you have sudden, severe abdominal pain that does not go away or keeps returning. Call 999 or go to A&E if severe pain spreads to your back, you have difficulty breathing, a fast heartbeat or persistent bloating, or you feel seriously unwell. Do not drive yourself. Take your medicines, or a current medicines list, with you.
Tell the clinician that you take Mounjaro or another GLP-1 medicine. Privately prescribed medicines may not always be visible in an NHS medication record.
What the Current UK Safety Information SaysThe January 2026 MHRA safety update strengthened warnings for all GLP-1 receptor agonists and dual GLP-1/GIP medicines. It advises clinicians to remain alert to acute pancreatitis, including rare reports of necrotising and fatal cases. This does not mean that severe pancreatitis is common. It means the symptom pattern must be recognised quickly.
| Medicine | Current UK product information | Relevant gallbladder wording |
|---|---|---|
| Mounjaro (tirzepatide) | Acute pancreatitis is listed as uncommon. It has been reported during treatment, including serious post-marketing cases. | Gallstones and inflammation of the gallbladder are listed as common adverse reactions. In weight-management studies, acute gallbladder events were associated with weight reduction. |
| Wegovy (semaglutide) | Acute pancreatitis is listed as uncommon. In the 2.4 mg phase 3a programme, adjudication-confirmed acute pancreatitis was reported in 0.2% of participants receiving semaglutide and in fewer than 0.1% receiving placebo. | Gallstones are listed as common. In the 2.4 mg programme, gallstones were reported in 1.6% of participants and led to cholecystitis in 0.6%. |
| Foundayo (orforglipron) | Acute pancreatitis is listed as uncommon. Its SmPC instructs patients to seek immediate medical attention for persistent, severe abdominal pain. | Acute gallbladder disease has been reported. The SmPC advises diagnostic studies and clinical follow-up if cholecystitis is suspected. |
The figures above come from different product information and trial programmes, so they should not be used as a direct comparison of which medicine is safer. Reporting systems and study populations also differ.
Important distinction
A rise in amylase or lipase on a blood test, without the symptoms and other findings of pancreatitis, does not by itself confirm acute pancreatitis. Diagnosis is made by a clinician using the full clinical picture.
Your gallbladder stores bile, which helps your body digest fat. Gallstones can form without causing symptoms, but a stone that blocks a bile duct can cause severe pain. Gallstones are also one of the most common causes of acute pancreatitis because a stone can block the shared opening near the pancreas.
Symptoms that may point to gallstones or cholecystitis
- constant pain in the middle of your abdomen or under the ribs on the right
- pain lasting longer than 30 minutes and sometimes several hours
- nausea or vomiting
- fever, shivering or feeling acutely unwell
- yellow skin or yellowing of the whites of the eyes, dark urine or pale stools
NHS guidance advises urgent assessment through a GP or NHS 111 for abdominal pain lasting longer than 30 minutes. Call 999 or go to A&E for sudden severe pain, pain spreading to the back with vomiting, a very high temperature or jaundice.
Pancreatitis, Gallbladder Pain or a Common Side Effect?Common digestive effects
Nausea, reflux, bloating, constipation or diarrhoea can occur during dose escalation. They are usually mild to moderate and are not normally dominated by severe, unrelenting pain.
Possible pancreatitis
Severe and persistent upper abdominal pain, often spreading to the back, with or without nausea and vomiting. This needs urgent medical assessment.
Possible gallbladder problem
Constant pain in the middle or right upper abdomen, often lasting more than 30 minutes. Fever, jaundice or vomiting makes urgent assessment more important.
You cannot reliably diagnose the cause of severe abdominal pain at home. If the pain is severe, persistent or accompanied by red-flag symptoms, seek help rather than waiting to see whether it settles. For milder digestive symptoms without these warning signs, the broader Mounjaro side effects guide explains what is common and what may help.
Does Mounjaro Cause Pancreatitis?Acute pancreatitis has been reported in people taking tirzepatide, and it is included in the current Mounjaro product information. This establishes a recognised safety warning, but it does not prove that Mounjaro caused every reported case. Pancreatitis also occurs in people who are not using GLP-1 medicines, most commonly because of gallstones or heavy alcohol use.
The practical message is not to assume that every stomach symptom is pancreatitis, and not to dismiss the characteristic warning signs as a routine side effect. A clinician may use blood tests and imaging to confirm the diagnosis and look for other causes.
What happens to treatment? If pancreatitis is suspected, current product information says the medicine should be stopped. If the diagnosis is confirmed, Mounjaro, Wegovy or Foundayo should not be restarted. A prescriber should guide the next step rather than the patient switching or restarting treatment alone.
Mounjaro, Wegovy and Foundayo have not been studied in people with a previous history of pancreatitis, and their current product information advises caution. A history of pancreatitis is therefore something your prescriber needs to know before treatment begins. It is not a detail to leave out of an online consultation.
Other factors that may be relevant include gallstones or previous gallbladder disease, high triglycerides, heavy alcohol use and medicines or health conditions known to raise pancreatitis risk. Rapid weight loss can also increase the chance of gallstones. These factors do not diagnose a problem and do not automatically mean treatment is unsuitable, but they may change the individual risk assessment or follow-up plan.
Can You Prevent Mounjaro Pancreatitis? There is no proven way to remove the risk completely. Be cautious of claims that a supplement, special diet or routine can guarantee prevention. The most useful steps are practical:
- give your prescriber an accurate history of pancreatitis, gallstones, alcohol intake, triglyceride problems and other medicines
- follow the prescribed dosing schedule and do not increase the dose more quickly than advised
- avoid crash diets and discuss unexpectedly rapid weight loss with your prescriber
- recognise severe, persistent pain and seek help promptly rather than treating it as routine indigestion
- keep alcohol within UK low-risk guidance, or ask for support if reducing alcohol is difficult
These steps support safer treatment, but they cannot rule out pancreatitis or gallbladder disease. Do not delay urgent assessment while trying self-care measures.
What to Do If You Think Something Is WrongIf pancreatitis is confirmed, current UK product information says the medicine should not be restarted. Your hospital team and prescriber can discuss recovery and other approaches to weight management. If gallstones or cholecystitis are diagnosed, the decision about treatment depends on the cause, symptoms and clinical plan. Do not stop, restart or switch prescription treatment without individual advice unless an urgent clinician has told you to do so.
If gallbladder surgery or another procedure is planned, tell the surgical and anaesthetic team about every GLP-1 medicine you use. The separate guide to having surgery while taking Mounjaro, Wegovy or Foundayo explains why the timing decision belongs to the clinical team.
Reporting a Suspected Side EffectYou can report a suspected medicine side effect through the MHRA Yellow Card scheme. A Yellow Card report does not replace medical care. Seek urgent help first when symptoms are severe.
