A true plateau is generally defined as little or no change in weight over 3–4 consecutive weeks, despite consistent adherence to your medication and lifestyle plan. It's different from the normal week-to-week fluctuation everyone experiences due to water retention, hormonal changes, sodium intake or simply how full your digestive system is on the day you weigh yourself. If your weight is bouncing within a 1–2kg range from week to week, that's very likely normal variation rather than a genuine stall.
Why Plateaus Happen on WegovyWegovy (semaglutide) is highly effective at reducing appetite and slowing digestion, but it doesn't override basic physiology. As you lose weight, your body requires fewer calories to function — a smaller body simply burns less energy at rest. This process, known as metabolic adaptation, means the same calorie intake that produced weight loss early on may only maintain your weight later in treatment. This is expected, not a treatment failure.

In the STEP 1 trial, weight loss on semaglutide 2.4mg followed a clear curve: steady loss across roughly the first 60 weeks, followed by a natural flattening as participants approached their new, lower stable weight — this is the expected shape of the response, not an unusual pattern specific to individual patients. Plateaus most commonly become noticeable somewhere between 6 and 9 months into treatment, though timing varies considerably by individual, and can occur earlier for patients on lower maintenance doses.
A flattening curve on trial graphs isn't a sign the medication has "stopped working." Appetite suppression and slowed digestion are still active — the plateau reflects your body finding a new energy balance at a lower weight, which is a sign of progress consolidating, not reversing.
As appetite suppression continues, it's easy for small amounts of higher-calorie food or drink to make up a larger proportion of reduced overall intake. A brief food diary for 1–2 weeks can reveal patterns — such as calorie-dense drinks, grazing, or portion sizes — that are easy to miss otherwise.
Reassess activity levelsAs body weight decreases, the same walk or workout burns fewer calories than it did at a higher starting weight. Gradually increasing activity, particularly resistance or strength training to help preserve muscle mass, can support continued progress and overall body composition.
Check sleep and stressPoor sleep and elevated stress both affect ghrelin and cortisol, hormones that influence hunger and fat storage independently of Wegovy. Improving sleep consistency is a genuinely evidence-based lever, not just general wellness advice.
Discuss dose review with your prescriberIf you've been on the same dose for several months and diet, activity and sleep all look reasonable, it may be appropriate to review whether a dose increase — including the licensed 7.2mg option for appropriate patients — is suitable for you. This should always be assessed by your prescriber, not adjusted independently.
Give it timeSome plateaus resolve on their own within a few weeks as your body continues adjusting. Consistency often matters more than any single change.
- Don't increase your dose beyond what's prescribed, or more frequently than scheduled, to try to "break through" a plateau — this significantly increases side-effect risk without reliable extra benefit
- Don't drastically cut calories far below a sustainable level — this can trigger muscle loss and make long-term maintenance harder
- Don't combine Wegovy with other weight loss medications or supplements without medical advice
- Don't judge progress by the scale alone — measurements, how clothes fit, and energy levels are all valid markers of continued progress
Any change to your Wegovy dose or treatment plan should be discussed with your prescriber first. Self-adjusting dosing to try to overcome a plateau is not supported by the prescribing guidance and increases your risk of side effects.
- Waist, hip and other body measurements, which can change even when weight is stable
- How clothing fits
- Energy levels, sleep quality and mobility
- Blood pressure, blood sugar or cholesterol improvements, if you're monitoring these with your GP
- Relationship with food — reduced cravings, better portion control, fewer intrusive food thoughts
